Position Purpose: Are you ready to make a meaningful impact in the comfort of patients' homes? Join Mount Carmel Home Care as a Primary Care Nurse, where you'll use cutting-edge technology and clinical expertise to deliver exceptional care to a growing patient population. Specializing in IV and infusion therapy, you'll provide vital services to those who need it most, along with other essential home care services.

What You Will Do

• Deliver skilled nursing care with a focus on infusion therapy.
• Develop and implement personalized patient care plans.
• Educate patients and families on managing illnesses at home.
• Coordinate with physicians and other healthcare professionals to ensure comprehensive care.
• Utilize industry-leading EMR systems to document patient care accurately and efficiently.
  
  

Minimum Qualifications

• Current RN license in the state of Ohio.
• Minimum of 2 years of nursing experience, with strong infusion therapy skills.
• Valid driver’s license and reliable transportation.
• Excellent communication and organizational skills.
• Ability to work independently and manage time effectively.
• Preferred experience in Home Care or Med/Surg Hospital settings.
